There is another alternative to the two options you mentioned, which is a variation on the open technique of repair, pioneered in Canada by Dr. Shouldice. The Canadian repair uses an overlapping tissue technique with permanent sutures. At Shouldice Hospital (in Thornhlll, Ontario--near Toronto) they give a lifetime guarantee against recurrences with their technique.

Shouldice Hospital is, to my knowledge, the only hospital in our hemisphere devoted entirely to hernia repair. I have had two procedures there myself, with good results. Surgeons trained at Shouldice have to be an assistant on hundreds of surgeries before they are allowed to lead a surgical team. A typical surgeon at Shouldice does over a thousand procedures in a year, so the training and experience is unusually rigorous. It shows in their statistics--recurrences are a fraction of that found with other techniques.
