Ultraviolet blood irradiation (UBI) was successfully used in the 1930s and 1940s for a variety of diseases. Despite this success, a lack of understanding of the detailed mechanisms of action and the achievements of antibiotics gradually reduced the use of UBI from the 1950s onward. The emergence of new viral infections, from HIV/AIDS to Ebola, from SARS and MERS to SARS-CoV-2, drew renewed attention to this therapeutic opportunity. UBI has a complex virucidal activity, acting primarily on the immune system response. It has effects on lymphocytes (T cells and B cells), macrophages, monocytes, dendritic cells, low-density lipoproteins (LDL), and lipids. Knott's technique has been applied to bacterial infections such as tuberculosis and viral infections such as hepatitis and influenza. The more complex extracorporeal photopheresis (ECP) is also being applied to hematologic cancers such as T-cell lymphomas.

This article describes an FDA Phase II clinical trial of ultraviolet blood irradiation (UVBI) in patients with hepatitis C virus (HCV) infection. The therapy was shown to be safe in this US FDA clinical trial. Two patients in the trial who had concurrent psoriasis showed a clinical response to this therapy. This device has also demonstrated efficacy in unpublished animal studies on influenza virus and simian immunodeficiency virus infection.
