The potential of vitamin D for cancer prevention and adjunctive treatment has long been bolstered by several discovered mechanisms involving the regulation of cell growth and differentiation, apoptosis, intercellular contacts, angiogenesis, immune function, and interaction with the gut microbiome. Figure 2 graphically displays the main mechanisms of action of vitamin D. These effects are primarily indirect results through the explained epigenetic and transcriptional changes, which involve the synthesis of transcription factors, chromatin modifiers, noncoding RNA (ncRNA), and microRNAs (miRs), through protein-protein interactions and signaling interference. Furthermore, vitamin D was considered a promising substance to counteract several carcinogenesis-related epigenetic changes, which increase the expression of tumor genes and decrease tumor suppressor genes.
Experiments with colon cancer cells confirmed these results and identified pro-apoptosis targets of VDR. Calcitriol induces the transcription of several pro-apoptotic genes, which encode or interact with Bcl-2 family proteins in ways that enhance mitochondrial membrane permeability and apoptosis. Rapid cell growth and extracellular matrix disruption are addressed by tumor cells with pro-vascularization signaling via VEGF, angiopoietin-1, and platelet-derived growth factor (PDGF) to meet their oxygen needs. An anti-angiogenetic effect of calcitriol was supported in human cancer cells and mice, in which calcitriol decreased VEGF expression and tumor vascularization was observed. Table 1.
